2. Available to employer-contributory groups down to 2 employees enrolling and to voluntary groups down to 5 enrolling
3. Orthodontia benefits for adults and/or children
4. Out-of-network UCR reimbursement at the 80th or 90th percentile
5. Dual option available

Cost-savings features include:

1. Optional Lifetime Deductible in lieu of Calendar Year Deductible
2. Oral surgery, periodontic and endodontic services that can be moved independently from Basic to Major services
3. Broader PPO Network: access includes TDA and DenteMax PPO networks
4. EyeMed discount plan included for dental groups who select orthodontia benefits
5. 5% discount on dental rates when bundled with another line of coverage
