Answers to Questions about In-Network Only Dental Plans

Q. Are there any waiting periods on this plan?

While there may be an employer-imposed waiting period that must be met before an employee is eligible to enroll, there are no waiting periods to use benefits covered by this plan.

Q. Do members need to select a provider before they enroll?

There is no need to select a provider before enrolling. Once a member is enrolled, he or she can make an appointment with any general dentist within our network. Each family member can select his or her own general dentist from our directory.

Q. Does everyone have to see the same dentist?

Each family member may select his or her own general dentist from our directory. To find a new dentist use the provider locator feature on www.yourdental.com/bhp.

Q. What if an employee needs to see a specialist?

With this plan, members may self-refer to any specialist within the network without needing any preauthorization.

There is no coverage for out-of-network treatment with the exception of coverage for emergency palliative relief of pain and orthodontics.

Q. What if an employee needs emergency care?

The Advantage Plus INO plan provides coverage for emergency care and all of our contracted General Dentists provide emergency dental services 24/7.

The employee should refer to the plan's Evidence of Coverage, which explains what to do when emergency care is needed.

Q. What is the PLAN MAXimizer?

Your INO dental plan includes the PLAN MAXimizer feature, which allows members to carry forward a portion of their unused annual plan maximum into an account for future use.

To access this feature, members must use their dental benefit at least once per year in order to accumulate awards. If no claims are submitted, no awards are earned.

If the total of all submitted claims paid for a member does not exceed the annual threshold amount, an award balance is established and will be available for the following year, in addition to the new year's maximum.

If all claims during the year are paid to network providers, members will also qualify for an additional $100 bonus award that will be applied to the annual plan maximum.

Awards are not accrued or cannot be used for orthodontic services.

Q. Does my plan include a Prenatal Dental Care Program?

Yes. We offer a prenatal dental care program that provides full coverage for cleanings, deep scaling, debridement, and periodontal maintenance for women in their second or third trimester. The dental care provided under this program does not apply to the INO plan's deductible or annual maximum.
